I have seen this with the three blades. My friend shot a deer and seen the same results you did. We came to the conclusion that he took a quartering shot. Since the head did not hit square this caused one of the blades to deploy before the other two basically locking them down.
Was your shot a quartering?
I contacted Field Logic with an email say I thought there was a design flaw with the three blades. They told me to send in my three blades and they would send me the two. So I gathered two of my friends and mine three blades (even the used and broken ones) and sent them in. They replaced them with new three blades which I took to Scheels and they traded me for the two blades.
I have switched back to Reapers I feel they are a much stronger and easier head to keep closed.
