Gun --> Remington SP-camo 10 ga.
Shells --> Federal #5's, 3.5"
Calls --> 2 homemade slates and 3 different strikers also homemade, 3-4 mouth calls (woodhaven custom calls)
Decoys --> All buckwings, bobbin head hen, 2 jakes (1 half struttin)
Ass pad --> most important, HS air pad
Camo --> Anything works as long as you are still and everything is covered.

Besides the things already mentioned (though some of my choices would be different as far as make/model/etc), don't forget your binoculars, a small saw or pruning snips for trimming brush, bug spray, an extra pair of gloves in case one pair gets wet and a camera w/ extra battery/film. There are other things I'm forgetting but hopefully this helps!
