If a buck's rack basically keeps getting wider all the way to the tips of the main beams, at what point do you measure the inside spread?

I think it is the widest point perpendicular to the head and I also think it cannot exceed the length of the main beams.

Right and if it does then you use the longest beam length for the spread measurement.
