Cross bow question

I had an accident and will not be able to pull my bow back. The Dr said that I need to fill out a document form and then he would sign it. Where can you get this? Also do I just buy a regular bow tag then? This is all new to me. Thanks.

It is on this page, print it, fill it out and have the Doctor Sign. Keep a copy and mail the original in.
