My stepdad killed one similar to that. It had two normal antlers in the right location but also a third small 1 inch point that came out just below the back of the eye.
I read a research paper that indicated you can actually get a antler to grow anywhere on a deer if the cell tissue from the pedicel area is transplanted. I have seen the same thing on antelope much more frequently. The will have small horns less than 1" long growing on their foreheads and the bridge of the nose.
I had pics of the buck where he did not appear blind in that eye. The taxi said he had a lot of bruising and pus around that eye and forehead as well as some deep cuts that he figured was from fighting.
